Kernig sign evaluates for meningeal irritation. With the patient lying supine, the hip and knee are flexed to 90 degrees. The examiner then slowly extends the knee and then repeats the maneuver on the opposite leg. A positive test occurs when the patient demonstrates resistance, pain, or an inability to fully extend the leg, typically occurring bilaterally.
